Just in: former Equifax CEO Richard Smith has submitted his opening remarks for Tuesday's testimony in front of the U.S House Committee on Energy and Commerce. Here are some snippets:

"As CEO I was ultimately responsible for what happened on my watch. Equifax was entrusted with Americans’ private data and we let them down. To each and every person affected by this breach, I am deeply sorry that this occurred."

In written testimony of the former Equifax CEO, he says he will recount for Congress and the American people, what happened when Equifax was hacked "by a yet unknown entity."

"....the investigation continues, but it appears that the breach occurred because of both human error and technology failures. These mistakes – made in the same chain of security systems designed with redundancies – allowed criminals to access over 140 million Americans’ data."

He also provided a timeline of warnings and errors and how they were made.
